Proposals Sought for Home, Community-Based Title III and Kupuna Care Services

The County of Maui Department of Housing and Human Services, Office on Aging, announced a Request for Proposals for Title III and Kupuna Care home- and community-based services.

Home health agencies, transportation organizations and related providers are encouraged to submit proposals for senior and disabled services to address the following needs:

1. Adult Day Care/Adult Day Care Respite
2. Chore
3. Family Caregiver Support Services
4. Friendly Visiting/Telephone Reassurance
5. Home-Delivered Meals Expansion
6. Homemaker
7. In-Home Respite
8. Legal Assistance
9. Money Management
10. Nutrition Counseling
11. Personal Care
12. Transportation Expansion

Service areas are within Maui County, including Lānaʻi, Molokaʻi, Hāna and outlying geographic areas.

An RFP orientation briefing will be held Tuesday, April 25, 2017 at 1:30 p.m. at Hale Mahaolu Ekolu, 717B Makaʻala Drive, Wailuku. Deadline for proposals is 4:30 p.m. on Wednesday, May 31, 2017 at the Maui County Purchasing Division, 2145 Wells Street, Suite 104, Wailuku.

Visit www.mauicounty.gov/purchasing and click on “State and County Procurement Notices” on the left to view RFP procurement rules and application. Proposals will be reviewed and the Maui County Office on Aging will select successful proposals based on the factors outlined in the RFP.
