A 27-year-old Kahului man died at the scene of a motorcycle accident on Kamehameha Avenue on Friday afternoon.  Maui police have since identified the operator of the motorcycle as Taylor K. Siverly Guido of Kahului.

The incident was reported at around 4:43 p.m. on Friday, Aug. 18, 2017 on West Kamehameha Avenue south of Alehela Street in Kahului.

According to police reports, the collision occurred as a motorcyclist, operating a white 2003 Kawasaki motorcycle and traveling north on West Kamehameha Avenue, lost control of the vehicle and fell onto the roadway.

The motorcyclist then slid on the roadway and collided into a white 2004 Honda Civic four-door sedan that was traveling south on West Kamehameha Avenue, and was making a left turn onto Alehela Street.

Police say the male operator of the motorcycle sustained fatal injuries and was pronounced dead at the scene.  The operator of the Honda Civic, a 45-year-old man from Kahului, was not injured in the collision.

Police say a preliminary investigation reveals that Siverly Guido was not wearing a helmet at the time of the collision.  The operator of the Honda Civic was wearing his seatbelt, according to police.

The involvement of speed, drugs and alcohol are still pending as the investigation is ongoing.

This was Maui County’s seventh traffic fatality for 2017, compared to 11 the same time last year.  It was also the third fatal traffic accident this week on Maui County roads.  On Tuesday, Aug. 15, a California woman suffered fatal injuries in a moped crash on Mākena Alanui Road in South Maui; and on Monday, Aug. 14, a Haʻikū man suffered fatal injuries in a single vehicle roll-over crash on Hāliʻimaile Road on Monday night.
