The Maui County Mayor’s Office of Economic Development (OED) is now accepting invoices from vendors who provided goods and services under the recently terminated grant titled “250th Celebration of Queen Ka‘ahumanu” (G4607) and have not yet been paid in full, a Maui County press release stated on Oct. 22, 2018.

Invoices must be complete with full contact information and detailed description of services performed or goods purchased. If vendor provided consulting, hourly rate and number of hours worked must be on the invoice. All submissions must provide proof that work was authorized and evidence that work was completed. Vendors must also provide a W-9 Form with their submissions.

The deadline for submissions is Nov. 16, 2018 by 4:30 p.m. Submit all documents to [email protected] and include “G4607” in the subject line of the email.

Any payments disbursed from the County of Maui will be based upon verification and balance of funds available under G4607. OED will contact claimants if additional supporting information is needed.
