The incident was reported at around 5:22 p.m. on Wednesday, January 11, 2012, near a residence on Airport Loop in Hoolehua.

According to police reports, the boy had stepped off the tractor when it came to a stop.

A preliminary investigation reveals that the boy’s grandfather, Walter Mendes, reached out to grab the boy and pull him back onto the tractor; however, his foot slipped off the clutch which caused the tractor to move forward, and run over the boy.
