Police say they received a 9-1-1 call at around 3:57 p.m. on Monday, Oct. 5, 2015 reporting the incident on Kulawai Loop in Kaluakoi on the west end of the island.

According to a report filed by Maui Police Captain Ricky Uedoi with the Molokaʻi Patrol District, Alavazo was in the company of two friends, also described as Molokaʻi residents, during the incident.

Police say Alavazo was treated by personnel from the Maui Fire Department and Emergency Medical Services before being transported to the Molokaʻi Airport and flown to the Maui Memorial Medical Center where he later died on Tuesday afternoon, Oct. 6, 2015.

Capt. Uedoi said Molokaʻi Police recovered the gun from the incident.  The type was not immediately available.

When asked to whom the firearm was registered to, and if anyone at the scene had a permit to carry, police said those items remain under investigation.

Police expect that there will be related cases generated as a result of the investigation.

Police say that at this time, the incident remains under investigation and has been classified as a miscellaneous accident.
