The Location:Hale Makua Health Services is a Maui based non-profit that improves the well-being of those in its care through compassionate personalized health services. We are devoted to helping Maui's Elders live their best lives. Hale Makua's Wailuku location is a 124-bed nursing facility.The Position:We're looking for passionate Social Services Assistant to provide range of services to community residents and families from the time of admission, during their stay and through discharge appropriate to the resident's needs.
Completes resident social histories, discharge plans, and also develops care plans as part of the inter-disciplinary team.
Ensures on-going psychosocial assessment and assists residents with access to additional services as needed throughout their stay at Hale Makua Health Services.
Requirements:
Graduate from a recognized college or university with specialized training or degree in human services field including but not limiting to sociology, special education, rehabilitation counseling and psychology or equivalent combination of education and experience.
One year of experience in a health care setting working directly with individuals.
Current and valid 2 Step TB clearance and health review (can be done in-house).
