Police released information today saying drugs and alcohol were determined to be contributing factors in a fatal motor vehicle crash reported over a month ago in South Maui.

The incident was reported at 7:58 p.m. on Thursday, June 20, 2013, on Mapu Place, about 90-feet east of the Mililani Street intersection in the Maui Meadows subdivision.

Maui police say 36-year-old Sirius Fazao Pereira of Wailea was initially transported to the Maui Memorial Medical Center in critical condition, but succumbed to his injuries and died on Thursday, July 4, 2013.

According to police reports, Pereira was traveling eastbound on Mapu Place when the vehicle he was operating drifted off the roadway and collided into a parked trailer on the eastbound shoulder of the road.

The incident was the 10th traffic fatality on Maui County roads this year.
