Under the general direction of the Clinical Supervisor, the staff member is responsible for providing a range of mental and behavioral health services to youth and families.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide intake, assessment, safety planning, case management, transition, and discharge planning to participants.
- Identify, develop, and implement individual service plans that specify goals, objectives, and measurable outcomes.
- Provide clinical services to clients and families.
- Deliver individual, family, and group therapy to participants and their families.
- Coordinate with external agencies, schools, and community resources to ensure comprehensive care for participants.
- Coordinate with care coordinators, providing progress reports and recommendations for discharge planning or continuation of treatment.
- Participate in all IEP and treatment planning conferences as requested.
EDUCATION AND/OR EXPERIENCE:
1. MSW or graduate degree in a related field.
2. Two years of experience in direct practice in individual, group, and family counseling.
