The fee is $10 for senior citizens 65 years and older, and $15 for all others, payable in cash only. After processing, the cards will be mailed to applicants within two to three weeks.

Priority will be given to applicants with completed application forms and required certified documents. State officials say applications without these documents cannot be processed.

New applicants must provide an original social security card and a certified copy of their birth certificate.

All changes in status or name due to marriage, divorce, annulment, adoption or citizenship must also be supported by certified government-issued documents. State officials say altered and illegible documents are not acceptable.

Recorded information on application requirements is available at (808) 587-3111 and on the internet at www.stateid.hawaii.gov.
