Maui Health Expands COVID-19 Vaccine Clinic to Qualifying Maui Residents
CDC identified Phase 1A and 1B can now receive vaccine at Maui Health
Maui Health announced today that it is expanding its COVID-19 Vaccination Clinic to include vaccinations for Hawaiʻi residents that fall within the CDC identified Phase 1A and Phase 1B vaccine prioritization timeline.
The Maui Health COVID-19 Vaccination Clinic is held in the main lobby of Maui Memorial Medical Center located at 221 Mahalani Street in Wailuku. “We are excited to offer this life-saving vaccine to Hawaiʻi residents,” said Mike Rembis, Maui Health chief executive officer, “it is critical we do everything we can to help vaccinate as many of our residents as possible to help stop this virus from causing additional harm.”
The Vaccine Clinic is open seven days a week from 7:30 a.m. to 8 p.m. and will begin accepting appointments today for these identified groups.
Maui Health COVID-19 Vaccine Clinic Step-By-Step Registration Instructions
The Maui Memorial Medical Center Vaccine clinic is open Monday to Sunday, by appointment only.
Starting Friday, Jan. 8, 2021, Maui Health will provide vaccinations to community members who are included in the Centers for Disease Control (CDC) identified Phase 1A and Phase 1B vaccine prioritization timeline. This includes Hawaiʻi residents who are:
- Healthcare Workers
- Frontline Essential Workers (click for list)
- Adults ages 75+
If you qualify and would like to receive the vaccine at Maui Health, please follow these steps:
STEP 1: Complete the online COVID-19 Vaccine Appointment Request Form
- After you’ve submitted the appointment request form, you will receive an email from VAMS ([email protected]). If you do not receive an email from VAMS within 48 hours, please email [email protected].
STEP 2: Create an account with VAMS. The email from VAMS will include instructions on how to create your account in VAMS.
- For help with VAMS, download the VAMS Job Aid (www.mauihealth.org/covidvaccine).
- TIP: It is highly recommend using GOOGLE CHROME to register with the VAMS system.
STEP 3: Schedule your appointment in VAMS. After you register in step two, you can then schedule your appointment in VAMS.
- Choose the “Maui Health Location” in the VAMS scheduler.
STEP 4: Register with V-Safe before you arrive for your appointment. (www.cdc.gov/vsafe)
STEP 5: Complete the Pre-Vaccination Questionnaire in VAMS.
- You will receive this from VAMS approximately 24 hours before your scheduled appointment. This must be completed prior to your appointment.
STEP 6: Day of appointment – arrive on time
- The vaccine clinic is located in the Maui Memorial Medical Center main front lobby.
- Complimentary valet service is available.
- Make sure to have valid identification and proof of employment (if health care worker/frontline essential worker).

Governor David Ige provided an update earlier this week on the state’s COVID-19 vaccination efforts saying healthcare workers, residents and staff at nursing homes are currently getting vaccinated. The second doses of the vaccine have started to arrive and will also be administered.
Additionally, first responders, frontline essential workers, and those 75 years of age and older are also beginning to be offered the vaccine.
